- There had been a heavy weight during the previous shift over the machine face which had left an open break near the face and some stone had fallen at it. Early in the shift the ripping was blown down close up to the face and the men then removed the ripping and fallen stone and built up the gate packs. The deceased was instructed to break into the face at the gate end and get under good roof again. For some reason he was about three yards down the face when a piece of stone 8 feet by 2 feet by 1 foot 10 inches fell out between the packs and rolled against him bruising him about the back and cutting him between the legs. He had not commenced to break into the face as ordered to. Died on April 5th.
