- Deceased was struck on the head and flung into the roadway by a sheave post which came out as he was letting a full tram of rubbish down his heading. The incline rope provided was of the full expected length of the heading and, to allow of the varying length of rope required during the drivage of the heading, the spare rope at one end was coiled and secured by glands just above the cap. In this instance the glands were evidently slack, as directly the full tram was released from the mooring chain which held it in position at the face the rope slipped through the glands, and, when the coil was exhausted, there was a sudden jar which pulled out the post with the result stated above. An examination would have revealed the fact that the glands were not tight.
