Apparently he was standing on a wooden ladder in a sub-distribution room while connecting some dead wires to some overhead bare conductors, when he received a fatal shock. The bare conductors were electrically charged with an alternating current at a pressure of 440 volts. Contrary to the Electricity Regulation 28, (Factory and Workshop Acts), he was doing the work unaccompanied, and apparently he had exceeded the instructions given to him by the chief electrician.
