- He was casting coals back from the face when a piece of stone fell from two slips. It struck him on the foot, which caused a wound which not considered serious until blood poisoning supervened on 10th July. He died from lockjaw on 11th July. The stone gave no warning, and the place had been examined three times by the deputy before the accident occurred.
