- Deceased was travelling outbye on the completion of his shift on an endless rope haulage road laid with double way where sets of 14 to 18 tubs drawn in both directions with a bogie at each end at a speed of a little over three miles per hour. In some way he was caught by a loaded set and as evidenced by the position of his lamp stick and portions of his clothing dragged a considerable distance and was found dead under the ninth tub of the set. The road was well provided with refuge holes.
