- Deceased was working in a rock head rising 1 in 4. He was filling dirt from a round of shots fired at the end of the previous shift 3 hours before and his mate was drilling with a hammer drill in the roof strata near him when suddenly apiece of rock 6 feet 6 inches long by 19 inches wide by 9 inches thick fell from the roof. Deceased was caught by the fall and sustained a fractured spine from which injury he died on the 18th int. The long side of the fall was close to the hole being drilled indeed the front of the hole was broken off by the fall. The roof had been frequently examined during the morning and was considered safe. Apparently the shots bad disturbed it, and the drilling completed the separation. In future, temporary supports will be set until the ground is ready for permanent girders.
