Fourteen men were being lowered to the Silkstone Seam from the surface, a distance of 340 yards in the top deck of a three-decked cage in the downcast shaft. Deceased was standing at the end, and when about half way down he appears to have fainted or been seized with dizziness and fell out of the cage to the bottom of the shaft and was killed. The cage was being run at the ordinary speed and nothing occurred to cause him to fall out.
