The top deck of the double decked cage had been changed, i.e. the full tubs had been replaced by empty ones. After the bottom deck was changed there was then a short interval during which deceased turned away from the shaft and talked to another lad. On receiving the required signal the brakesman put his engine in motion and the cage commenced to descend. Sunley turned to the shaft and seized hold of the sneck of the top deck of the cage and was pulled into the shaft. He had evidently forgotten that the tubs had been changed and the cage was ready to descend.
