- The accident was caused by the loose board on the top of the upcast shaft casing for closing the aperture through which the rope passed and had play room for surging being drawn up to the pulley and knocking the rope off it on to the gudgeon or axle the sudden bend and jerk being quite enough to account for the rope break. The rope was a good one breaking strain when tested afterwards 36 tons working load 4 tons original breaking strain of rope 40 tons. It had been working two years and four months. 8 killed.
