- Died January 30th. Deceased had been employed for a week taking the tokens off the empty tubs as they came from the creeper to the back side of the shaft where the flat sheets are laid so as to form a slight dish dipping each way 1 inch per yard. The tokens are looped on to a staple which in the case of all but 6 of the 600 tubs in use is placed in the left front corner of the tub as it approached deceased from the creeper and easily got at. One of the 6 tubs with the staple in the right front corner came forward and deceased although he had been cautioned against doing so was riding on the tub with one knee on the buffer as he reached over to the tokens he fell off and just then another empty tub standing at the far side of the dish moved to its centre and he was crushed between them. The Jury recommended that the position of the token staple be made uniform through out and the Local Inspectors made the same recommendation in their report.
