He had placed a hutch of "redd" on to the hoist cage at the surface to have it raised up to the level of the dirt bing, but failed to raise the safety catch on the cage, which prevents the hutch from coming out. When the cage was raised the hutch fell out and hit him on the head and killed him instantly.
