- A walling stage on which were 17 men, was being lowered by means of two ropes worked from drums on a worm winch; when the signal to stop was given only one rope stopped and the stage was canted and nine of the men fell into water at the bottom of the shaft, one being drowned. The failure of the winch to work properly was probably due to a pin holding the clutch lever coming out of place.
