Deceased was engaged near the shaft bottom uncoupling full tubs and arranging sets of four tubs at a time for the cage. He was lowering three tubs from a set of blocks 24 feet above the shaft blocks, when in attempting to uncouple the first tub before it reached the blocks, his head was crushed between the first sod second when the first tub struck the blocks. More tubs followed behind before deceased was discovered. His head was badly crushed. The uncoupling of full tubs when in motion was prohibited, each tub having only one inch of buffer.
