- The deceased and other three men were being lowered to the Flockton Seam for the purpose of repairing a pump, when the engineman was unable to stop the cage and it dashed on to the bottom of the shaft with great violence and seriously injured three of them. The deceased died the following day. The reason the winding engineman could not stop, the engine was that a fitter had been removing the spindle of a valve in connection with a sight feed lubricator and had placed a bucket under it to catch the water that was liberated and without noticing it the edge of the bucket was under the edge of one of the balance weights attached to the horizontal shafts of the reversing gear of the engine and when the engineman attempted to pull over the reversing lever in order to put steam against the engine the weight came on to the top of the bucket and prevented his doing so. Those balance weights have since been boxed in.
