While walking up a hard heading, having a gradient of 1 in 6, an empty tram, which was been lowered by a hauling engine, left the rails and ran into the top of the heading and crushed him. There did not seem to be anything to account for the tram leaving the rails except that a few yards above the scene of the accident there was a safety point, if this was open it would derail the tram.
