Inspection made and inquest attended. Deceased with three other persons got in a manhole to let the full journey pass uphill when a coupling broke and the tubs ran back and getting off the rails knocked out three bars opposite the manhole causing a fall of roof which killed the deceased. The manhole was not sufficiently set back.
