- Deceased was proceeding along a haulage road, dipping towards the shaft, while the haulage rope was in motion, to his work in company with two other workmen, when a loaded tub became detached from the rope and ran amain. All the men attempted to reach the nearest manhole, but deceased in the confusion went to the wrong side and stepped right in front of the moving tub and was crushed. He died from his injuries three months later. The men were late owing to having overslept. When they started off along the road the haulage had not begun, but it did so while they were on the way. They should have been at the inbye end of the road before the haulage was set in motion.
