Deceased was in charge of the engine for working the cleaning belts. While the engine was running he reached forward near level gearing to screw up a grease lubricator and was dragged into the bevel wheels and instantly killed. A notice near his engine forbidding the oiling of the machinery in motion appears to have been frequently disregarded by him.
