Deceased was illegally riding on a clip attached to two full tubs on the endless haulage rope in a dip, when his shoulder came in contact with a low ledge of roof and he was rolled over the top of the first tub and dropped on the coupling between them, sustaining severe injuries. Pneumonia supervened, and he died on 11th August.
