- Deceased's duty was to couple together sets of 20 empty tubs at a siding on the fiat of the Main hill endless haulage rope, from which point they were taken forward, clipped to the rope, to another siding. During a lull in tub traffic, a runaway full tub which escaped from a clipping-on place at the Last level, 217 yards outbye, caught him as he was entering a side entrance to a pump room. The tub continued on its course for a further distance of 160 yards. The Monkey at the East level, which holds the tubs whilst being clipped on to the Main Hill haulage rope, failed to act and another Monkey 8.5 feet below was knocked out of action. The refuge holes were on the side on which the empties stood on the rails. The siding was lighted by five overhead electric lamps. Deceased was severely injured, and died from Septic poisoning on 16th October.
