He had holed a length of about 6.5 feet to a depth of about 1 foot under the roof coal, and had set one sprag 1 foot 6 inches from the open end of the holing, when the undercut coal fell upon him, fracturing his spine. An additional sprag should have been set. He died on the 27 June.
