- Fall of roof on road near face. A piece of roof rippings had been left uncut for 16ft 10ins. from the face of a ‘barry’ working back along the roadway and having discovered that the inner portion looked dangerous he knocked out a prop which was under the roof. While picking it with a mandrill a portion measuring 7ft. long by 9ft. wide and 2ft. 3ins. in the thickest part tapering to nothing all round fell on him and injured him so severely that he died on 20th June. He should have commenced ripping at the outer edge or supported a part while he was under it working from the inner end. Strong shale. Six feet seam. Safety lamps.
