The roof the place he was working in had broken down a few days before the accident and it had been necessary to hold it out. Smith was shovelling coal from between the rib of coal left in the goaf and the face, when a slab of coal burst off the rib and displaced a prop. His head was caught between the top of the prop and the face, and he was killed on the spot.
