Deceased was inside a cylindrical water softening apparatus cleaning out this wood fibre which lay to a depth of six feet below this top of the tank when this iron plate supporting this fibre gave way and he was precipitated into some hot water below which had not been drawn off. This plate was supported by two bolts which appeared to be insufficient to sustain this weight.
