Deceased was holing in the Red Mine ironstone seam when a fall of bottom stone occurred, crushing his head so severely that death was instantaneous. He was at the buttock, where a shot had been fired on the previous day and for which the sprags had been drawn. Some of them had been reset, and Skerratt had prepared a place for another, but continued holing while the loader went for a sprag.
