Crushed between a tub which ran too fast on to the landing and the brake frame. Tubs were jigged from an intermediate level as well as from the top of the dip and for this purpose a piece of wood lagging was put on the drum to regulate the running of the rope. Deceased had by mistake put the lagging on the drum when jigging a tub from the top. This caused the ascending tub to land on the plates before the empty tub was at the bottom.
