- Deceased climbed inside the fencing round the flywheel and connecting rod of a belt-driven shaking screen for the purpose of oiling the bearings. He was apparently caught by the flywheel and fell on the floor, and one leg was drawn under the shaft of the flywheel between the wheel and a pedestal. The log was severely injured, and death ensued on February 11th. It was neither necessary to oil the machinery while in motion, nor to climb inside the fence to do so. The accident was directly due to a breach of Special Rule 156.
