- Deceased was assisting to put a pack on at the wayend of a stall in the Shallow pit Coal seam, when a sudden fall of roof coal, followed by a large quantity of dirt or "bass," occurred. Deceased was knocked down and buried, and was dead when extricated six hours later. The fall was apparently due to a "weight" which caused a wayend bar to push out, and to a "slip" in the roof coal, to provide against which a wood chock had just been built at the opposite side of the wayend.
