Screening Plant - Deceased was assisting to get a load off a gravity hoist. The man at the brake left it thoughtlessly to assist also. When the load came off the empty cage shot up and deceased’s head was caught between the bottom ring of the cage and a girder forming part of the framework of the hoist. An empty tub had been left on the top cage.
