He was engaged with a sinker fitting pipes in a well shaft when, on being raised in accordance with a signal, he fell from the scaffold to the bottom, a distance of about 38 feet. The scaffold was built off the cage top and, at places, the clearance on the brickwork was 24 inches, 26 inches and 31 inches. There was no protection as required by General Regulation 176.
