He was assisting to renew a new shaft, weighing about 7.5 cwts., on to the coke ramming machine. The chain carrying the shaft broke, due to a faulty link, and the shaft fell on to deceased's leg and fractured it. Blood poisoning supervened and he died on 12th August.
