Details of Event
His duty was to put on the tubs at a mid-inset in a shaft where the work was intermittent. He was alone, but was heard to fall down the shaft. The fence gate was open and it is probable that he had forgotten to close it after landing a tub and afterwards inadvertently walked into the shaft.
