Deceased apparently drew the top cage too high, thus causing the shoe to stick above the guide and bring the cage up. The gate would also be lifted with the cage, leaving one side of the shaft unfenced. He fell down the shaft, but the evidence is incomplete, and it is not clear whether he went through inadvertently or otherwise.
