With a pair of horses the deceased was taking three wagons to the siding where the ironstone wagons from the quarry are tipped down shutes into them. He had reached the siding where the gradient is practically flat and while the wagons were in notion he unhooked the horses' chain, dropped it on the ground, and in stepping out of the four-foot way he stumbled and fell across the rail. The wheel caught and crushed him over the heart, killing him instantly. He was accustomed to the work.
