Deceased had just signalled away a loaded cage and was reaching to close the fence gate at the mouthing of the seam, when he was struck by full tubs and knocked into the shaft. He fell 35 yards to the shaft bottom and was killed. A set of full tubs had been allowed to pass too quickly through the "squeezers" a few yards away, and these ran into the standing tubs at the inset, which were not held up by any form of catch.
