While stepping on to the cage at the pit bottom to r elease a full tub which had become derailed, he was struck on the head and killed by a piece of coal which fell from a tub on the top cage. The clearance between the cage and the fencing placed at the bottom to protect the men working there from falling coal, etc. was only about 5 inches.
