He was attending to some tubs in the endless rope downbrow when he got fatally crushed by two full tubs end a timber trolley running back downbrow, off the flat landing place top of brow, after they had been unlashed which happened through the haulage engineman having a signal to reverse and the weight of the rope carried them back sufficient distance to run downbrow themselves. A warrick will be placed top of downbrow to prevent them going back.
