- Deceased was assisting to detach a set of four full tubs from the haulage rope, during a temporary stoppage of the rope. While two men attempted to take off the lashing chain, deceased apparently went between the second and third tubs to push them up and so slacken the chain. While he was in this position the rope re-started and he was crushed between the top of the last tub and a roof bar. There was only 3 inches clearance between the top of the tub and the bar.
