- At the commencement of the afternoon shift he, with the landers permission, was stepping into the bottom landing, when suddenly it was lowered by the engineman. Ryan was caught by the upper part of the bottom deck and pinned by it to the floor at the edge of the shaft. The engineman declares he received the signal to lower the gig and the lander strenuously denies having given a signal. There was no evidence of a signal, if given, having been given in any way other than by the usual means. Contrary to the requirements of the Special Rules, the lander, on this occasion, gave consent to the men to enter the "gig" before it had been sent on a trial run through the shaft. If keps had been provided at the surface level for supporting the cage when at rest, and used when persons were entering the cage, the accident would have been avoided.
