- The haulage rope in a tunnel dipping 1 in 4 was stopped as usual at 2 p.m. and four men started to ravel up the tunnel at the end of their shift. Shortly afterwards the rope was re-started by the man in charge at the top of the mound in order to move some empty tubs from an unsafe position. The starting of the rope caused a lashing chain, attached to a set of three full tubs to break. The tubs ran down the tunnel and struck deceased, inflicting injuries from which he died the same day.
