Deceased had descended in the cage to the bottom of the sump to clean under the weights and levers of the fans. Some men came out to go up the pit the hitcher told him of this and asked him if he was safe he replied that he was. They signalled the cage up with the men in the top deck heard him cry out and stopped the cage. Deceased was found crushed between the cage and the landing. There must have been a misunderstanding between deceased and the hitchers.
