Deceased was holing in the centre of the Ravine Seam, kneeling on bottom coal which had been loosened by shots. While in this position some top coal and overlying "lay" fell over on to him and caused instant death, his to skull being fractured. The accident was due to the failure to set sufficient sprags to the top coal, and for a distance of 9 feet there was no sprag.
