A set of three full tubs was being raised in a brow rising 1 in 4, when the tubs were momentarily held until the backstay was put on. Before the drag was placed above the tubs, the set ran back and caught the deceased who had just come to the landing. It was found that the link next to the tub drawbar was stiff, and it appears likely that the hook at the end of the haulage rope had not properly been put on.
