- Deceased and another man were loading a tub in the gateway of a small stall in bottom portion of Thick Coal seam alongside a fault when there was a very heavy bump and a piece of the rock roof, about 7 feet long, 3 feet wide, and 2 feet 9 inches thick fell from between two timber cogs. Deceased was kneeling against one of the cogs and his skull fractured. Two posts set under the stone were knocked out and the other timbers broken. There was a slip, previously unseen, on each side of the stone which fell.
