- Deceased's duty was to couple the tubs at a landing at the inbye end of a main and tail rope haulage road. He had come to the pit without his bait and was going outbye to get it when a set of 22 full tubs started against the regulations he was riding on the rope behind the last tub and was told to get off by the landing keeper who was at the rapper at the outbye end of the landing he got off and directly afterwards on a part of the road rising outbye one of the links of the loose coupling between the first and second tubs broke and 21 tubs began to run back; the road was very low and he was unable to get clear was forced back and refuge hole was not provided within the prescribed distance, although had there been one it is doubtful if it would have saved him. The coupling was made up of two end links of three eighths-inch and smaller intermediate links of iron. It was one of the small links that gave way and it was found in two parts. The iron seemed good. The smaller links are cut off a chain as required, the large links being fitted on to them at the colliery. The smaller links stand a test of 4tons 12cwts. 2qrs. and the manager estimated the strain they would be subject to at the time of the accident as 3,000 lbs. The Local Inspector examined the locus but made no report.
