- Through the jig rope chain getting somehow unhooked from the empty tubs at bottom of self-acting jig when starting jigging, it caused the full journey of tubs to run into the main haulage level, striking the endless rope running at right angles, and the jerking of the endless rope fatally injured the two deceased persons, in the main haulage level. 2 killed.
