A cage load of men had been drawn up the shaft and lowered on to the keps at the ground level. The opposite cage was empty and was suspended in the shaft 27 feet from the bottom. As the men got off the top cage thereby reduced its weight, the cage crept away, and deceased was caught between the cage and the shaft side whilst stepping out. He was doubled up and fell down the shaft. The winding engine was in good working order, but the brake had evidently not been put on sufficiently. The engineman had had an overwind previously, and he said he was thinking about the setting of the controller at the time, though he put the brake into the usual position.
