Crushed by cage in sump of shaft when trying to get out of cage. Engineman lowered the cage containing the undermanager and seven others beyond the usual getting-out place, into the sump. Owing to a breakdown, the engineman had been on duty 34 hours. No bell to indicator to warn the engineman.
