Rigby was fastening the bottom door of a slack wagon standing over a hole where it delivered to the elevator of washing machine when a loco with seven trucks came towards him on a slight incline. The trucks should have been stopped before reaching the slack wagon but the brakes and scotches applied were insufficient to stop them and there was a collision by which Rigby was killed. A stop block to protect the slack wagon was advised.
