A piece of the shaft timbering fell down and struck him at the platt or mouthing where he and others were filling ore into a kibble for hoisting. As the kibble was about to land he stepped forward to guide it on to the platt and as he did so a piece of timber struck him on the shoulder causing injuries which proved fatal the same evening. The shaft an old one, does not appear to have been examined since it was fitted up for hauling purposes some months before which seems a serious omission where men were employed in filling stuff or working at the bottom. As it was not used for men to ascend or descend the shaft men whose duty it was to examine it believed that it was not necessary.
